Have you been in close contact or in isolation with anyone that is suspected or covid-positive? 3. Have you travelled overseas in the last 14 days? 4. Has a temperature (38 degrees and above)? 5. Do you have any of the following symptoms? a. Cough b. Sore throat c. Shortness of breath d. Runny nose, sneezing, post-nasal drip (coryza) e. Lost of smell If you have answered YES to any of the questions above, we will contact you to discuss appointment options for you.
